Last week on Inside EMS, we discussed the radio report/pre-arrival notification, and this week, we discuss the bedside hand-off report.
Short version: It’s as unique as the patient, and the hospital, and the ED staff. There are a thousand variables.
But one good way to approach it is to wait until the entire care team is in the room so you have to give the report only once, and your hand-off report should include the same basic info as your written narrative; CHART, SOAP, SOAPIER, whatever format you use.
